Please read and review the entire Athletic Packet with your student. You must complete all of the forms and turn them into the Athletic Director or your student’s coach. ALL OF THESE FORMS MUST BE RETURNED AS SOON AS POSSIBLE. YOU MUST HAVE A CURRENT SPORTS PHYSICAL BEFORE YOU CAN START PRACTICE OR COMPETITION.
